In addition to the online profile, please also download and complete the following paperwork.

Submitted by you to Texas@DistinguishedYW.org:

• 2023 State Participant Agreement
• Health Form (please also include a copy of the front and back of your medical insurance card)
• Texas Talent Guidelines

Submitted by your school counselor:

• Academic Form (transcript and standardized test scores also required)

The Academic Form requires your school counselor to verify and fill out a portion. Please read the instructions in the document carefully – some portions should be filled out by you and are only verified by the counselor. Your counselor should email to Texas@DistinguishedYW.org or mail directly to the address in your info packet.

Paperwork for the 2023 Texas Program is due on December 10, 2022, and can be emailed to Texas@DistinguishedYW.org or mailed to the address included in your info packet.
